CMI's Test Authorization Slip Program

CMI's Test Authorization Slip program is an application designed to assist our clients in the ordering process of physicals, drug and alcohol tests. Using this program allows you to quickly and accurately enter your requests and submit them knowing that the information entered will ensure that the test results are processed and reported quickly.

 

Use of this program helps eliminate errors that can occur by not using the test authorization slip or by illegible requests.

System requirements

Operating System: Microsoft Windows 2000 or newer

Email Account. Our program will work with any MAPI compliant email program or you can use the programs internal system to send the information to us through your email provider or ours.

Printer: Any printer capable of printing landscape on 8.5" x 11" paper

Internet access: Internet access

 

Installation Instructions

Before you get started, make sure to close any program you have running except your web browser that is displaying these instructions. Also ensure that you have installation rights on your computer.

 

· If you have version 2 or older installed, you must uninstall the older versions.

· Microsoft Data Access Components 2.8 and .NET Framework 2.0 must be installed on your computer. If they are not they will be installed when the program is installed.

 

· Click on the Test Authorization Slip Program Installer link below (you must be logged into the site to see the link). The File Download dialog box will appear.

· Select Save and choose the folder location where you want to save the installer. (Save the installer to a know location on your computer, for example, to you desktop).

· Click Run. When the download is complete run the installer by either selecting Run on the Download Complete dialog box or double-click on the installer icon.

· If you receive a Security Warning dialog box select Run.

· The installation process can take a few minutes.

· Once the installation is complete, the program will start. You will be prompted to ender your activation code. If you do not have your activation code please call CMI at 800-840-1070 to obtain the activation code. Once you receive the activation code, enter the code and press OK to continue.

· The program will verify your activation code and download the database you need to run the program.

 

· Once it has downloaded the database the program will display the main screen of the program. At this point your program will now run normally.

Known Problems

If you see a prompt asking you if you want to "unblock" the program please select the "unblock" option. This will allow the program to access the internet to obtain program and database updates.

If the program informs you it is unable to send your request to CMI or it displays an error and you are using a MAPI compliant email program, this is due to the fact it is unable to locate the MAPI compliant email program or unable to access the email program. If you have an compliant email program please close the email program and the Test Authorization Slip program, then restart both programs. This should correct the problem. If this should fail you can try the programs internal email protocol or fax the authorization slip to CMI at 210-967-9233.
